The 18 million adults in England living with long term and multiple health conditions are currently failed by a health system that treats people in biomedical silos and which reacts to crisis instead of anticipating and preventing it. This results in poorer outcomes for people who experience the greatest burden of ill-health and an unsustainable growth in demand on the NHS.
Our new report shows how a joined-up service that is proactive, equitable and centred around people can be delivered in neighbourhoods, providing practical steps that local leaders can take now.
To achieve this shift, neighbourhood teams need to be cross-sector, bringing together all the agencies involved in a person’s life – health, social care, voluntary sector, housing and other agencies. We also need to value and invest in non-clinical roles who have the time to address the interaction between social and clinical needs, and help people regain agency over their own health. National leaders have a pivotal role to support local delivery by unlocking barriers and setting clear expectations of local systems to deliver person centred services.
